There are different kinds of scars that are specific to the type of skin. Each of these acne scars has a particular laser treatment for optimum results. They are as follows:
1. Ice Pick Scars
These acne scars are small deep holes found on the surface of the skin formed due to acne. They measure 2mm or less in length, are narrow, deep and depressed, making your skin look like it has been punctured with a sharp object.
Treatment Plan:
A technique called ‘Chemical Reconstitution of Skin Scars’ (CROSS) is performed with Trichloro Acetic Acid (TCA) as a Laser Treatment for acne scars/back acne/red acne scars. A high concentration of TCA is applied on the scars and left in place. This causes a slight burning sensation like an ant bite sting. Since our skin type has a lot of melanocytes, you may experience some post-inflammatory pigmentation, but it is only temporary and will fade with the application of prescribed creams.
The procedure takes about 2-15 minutes and normally, 3-4 sessions of TCA CROSS are needed to fill the scars. With each treatment, you will see a 20%-30% improvement. After 3 treatments, 80% improvement is apparent. Strict sun protection is advised for a week after the procedure and moisturizing sunscreen (SPF 30 or above) for 3 months post procedure.
How is the treatment performed?
Typically, a high concentration of TCA is applied on the scars with a toothpick and left in place.
Is it painful?
There is a slight burning sensation like the sting of an ant bite.
How long does it take to perform the procedure?
2-15 minutes, depending on the number of scars and open pores.
How many treatments will I need?
Normally, 3-4 sessions of TCA CROSS are needed to fill icepick scars.
What is the downtime following this procedure?
The skin starts peeling between Day 2 and Day 5 of the procedure. Strict sun protection is advised for a week post procedure, and a moisturizing sunscreen is advised for 3 months thereafter.
What is the success rate of these treatments?
With each treatment, expect a 20-30 percent improvement in the scar. Scar depth reduces even after several months post procedure. After 3 treatments, approximately 80 percent improvement is apparent.
Can this technique be combined with other aesthetic procedures?
Yes, it can be combined with lasers and fillers for optimal treatment, depending on scar type, depth and skin type.
Will there be any side effects after the procedure?
Ethnic skin type/ Asian skin type has a lot of melanocytes. So you will experience some post inflammatory hyper pigmentation. But this is only temporary and will fade away when specific creams are applied.
What post-procedure care is required?
Typically, patients have to use a sunscreen with SPF of 30 or above, twice or thrice a day and a skin lightening cream containing hydroquinone for a week after the procedure, to bring down the discolouration.
The advantage of this temporary discolouration is that filling ice pick scars post-treatment is easy.
2. Tethered Scars
These are most commonly seen on the temples, forehead and cheeks, and can be identified when the skin is stretched. These scars are attached from the base of the skin to the underlying muscle layer.
Treatment Plan:
A procedure called Subcision is best suited for tethered scars. This is a minor surgical procedure for treating depressed scars and wrinkles. It is performed using specialized needles filled with Platelet Rich Plasma (PRP) to lift the scar up. It is not painful since an anaesthetic cream is always applied 45 minutes to 1 hour before the procedure. The results are immediate and satisfactory. 2 or 3 sessions will give you up to 80% improvement.
How is the treatment performed?
It is performed using specialized needles. Fillers or PRP are injected to act as a spacer and lift the scar up.
Is it painful?
An anaesthetic cream is always applied 45 minutes to 1 hour before the procedure.
Can it combined with other procedures?
It can be combined with fillers of PRP. A large, tethered boxcar can be fractionally ablated too, after subcision. If the scars are atrophic, we have to combine it with fillers.
What results can one expect?
Results are immediate and satisfactory. 2-3 sessions derive up to 80 percent improvement.

3. Rolling Scars & Box Scars
These acne scars are round/oval depressions with steep vertical sides which give the skin a pothole-filled appearance.
Treatment Plan:
To treat this condition, the Fractional Ablative Laser UltraPulse Fractional Resurfacing is used in varying densities, depending on the depth of the scar. In this acne scar treatment, laser lights are targeted on the top layer of the skin and removed. The deeper scars are treated with the deep Fx of the UltraPulse, and then the surrounding edges are sculpted with the Active Fx.
How is the treatment performed?
The procedure takes around 30 minutes, followed by about 4 days of recovery time. Practically no pain is experienced because the area is numbed with an anaesthetic cream an hour pre-procedure. The recovery is so smooth that just applying a cream prescribed, restores healthy skin. During this time, you are advised to avoid direct sunlight. Results are visible within 4 months and you may not even need a second treatment. It may last up to 2 years, after which a skin booster or PRP may be required to keep up with ageing skin.
How are these scars treated?
The fractional ablative laser (UltraPulse) in different densities and energies is used, depending on the depth of the scars is used. In this procedure, micro fractional columns of laser light are targeted on the top layer of skin and removed. The intervening skin acts as a reservoir for supplying collagen to the neighbouring targeted skin, thereby producing collagen remodelling and healthier skin.
Is there any downtime for this procedure?
There is a downtime of 4 days for this procedure, but it is so subtle and smooth that just applying a “healing” cream restores healthy skin. During this time, the patient is advised to avoid direct sunlight. If the patient does want to go out, a prescription sunscreen is to be applied.
How long is the procedure?
The procedure takes around half an hour. The deeper scars are treated with the deep Fx of the UltraPulse, and then the surrounding edges are sculpted with the active Fx. Later, the over-lying skin is targeted to stimulate the stem cells at the dermo-epidermal junction. Practically no pain is experienced because the area is numbed with an anaesthetic cream an hour before procedure.
When can I expect to see results?
A lot of collagen remodelling and healthy cells are produced one week after procedure for up to 3 months. Results are visible within 4 months. Just one single treatment can produce 40 to 80 percent improvement (depending on the depth of the scar and the skin type) – you may not even require a second treatment. But if the boxcar is very deep, a punch graft may be required at a later stage.
How long does the effect last?
The results last for up to 2 years, after which a skin booster, PRP, or fillers may be required to keep up with the physiological ageing of the skin.
4. Atrophic Scars
These are indented scars formed when the skin is unable to regenerate tissue which leads to imbalanced scarring. Atrophic Scars are often the result of severe acne or chicken pox.
Treatment Plan:
Fillers are the best acne scar treatment for Atrophic Scars since lasers can help only partially. Fillers are injected at the right depth to lift the scars. Since the filler injections are preloaded with Lidocaine (anaesthetic), you will experience only mind discomfort. The results are instant and remarkable. No heavy, strenuous work should be done 24-hours post procedure and a bland moisturizer must be applied on the top layer. There are no side effects to this procedure. A touch of Filler will be required every 15-18 months, as Hyaluronic Acid fillers degrade in the body over time.

Lasers can help atrophic scars only partially; fillers are the best treatment. They are injected at the right depth to lift the scars. Results are instant and remarkable. A touch-up of the filler is required every 15 to 18 months, as hyaluronic acid fillers degrade in the body over time.
